日やけ止めだけではない、内側から肌を守るということ Beyond Sunscreen — Protecting the Skin Within

今週、ワシントン・ポストの記事を読んでいて、思わず目が止まりました。
基底細胞がんは最も一般的ながんのひとつで、アメリカでは成人の約5人に1人が罹患すると推定されています。40歳以降に多くみられますが、もちろん若い人でも無関係ではありません。
その記事で紹介されていたのは、23歳の女性のお話でした。彼女は鼻に小さなピンク色のカサカサした部分があることに気づき、ニキビ痕か湿疹だろうと思っていたそうです。しかし、実際には基底細胞がんでした。


私がこの記事を読んで特に驚いたのは、基底細胞がんが必ずしも見た目から怖いとは限らないことでした。小さなできものや傷、カサカサした部分など、一見すると乾燥肌や、なかなか治らないニキビのように見えることもあります。命に関わることはまれですが、周囲の組織へ深く広がっていく可能性があるため、早期発見がとても大切なのだそうです。

この話は私にとって他人事ではありません。私の弟も基底細胞がんを経験しています。
弟は大のゴルフ好きで、週7日と言っていいほどゴルフコースに出ています。

そのため今は以前よりずっと、外に出る時の日やけ止めを意識するようになりました。

でも、日傘だけでは十分ではありません。
皮膚科医は、SPF30以上の広範囲の紫外線を防ぐウォーターレジスタントの日やけ止めを推奨しています。また屋外では、およそ2時間ごと、そして泳いだり汗をかいたりした後には、日やけ止めを塗り直す必要があります。

ここからが、私が紫外線対策について特に興味深いと感じているところです。
SPFは、肌を守るための最初の、そして欠かせない防御です。でも肌に届いてしまうわずかな紫外線については、どう考えたらよいのでしょう?
この疑問が、私がプロテキシダインに強く興味を持つようになった理由のひとつです。



プロテキシダインについては、紫外線によって生じるさまざまなダメージを軽減する可能性について研究が行われています。その中には、紫外線を浴びた後にDNAに生じる損傷の一つ「シクロブタン型ピリミジンダイマー(CPD)」に関するものもあります。


これまでに発表されている研究には、ex vivo(生体外)の皮膚モデルを用いた研究も含まれています。
だからこそ、この期待される研究については、正確に、そして責任をもってお伝えすることが大切だと私は考えています*。(*体外の実験で良い結果が出たからといって、人間の体でも100%同じようにうまくいくとは限らないため、どこまでが分かっていてどこからがまだこれからの課題なのかを、誠実に正しく伝えたい。)

プロテキシダインを開発したトピックスファーマシューティカルズ社の研究開発責任者、スティーブ・ヘルナンデスさんが、以前この考え方を「3つの防御ライン」として説明してくれたことがあります。

第1の防御:
日やけ止めで、できる限り紫外線を防ぐこと。

第2の防御:
それでも肌に届いてしまった紫外線によって生じる酸化ストレスに、抗酸化成分でアプローチすること。

第3の防御:
紫外線によるダメージに対して、より深い細胞レベルで肌をサポートすること。

私はこの説明がとても気に入っています。とても複雑なことを、わかりやすく整理してくれるからです。
そして、ここで大切なのは、プロテキシダインは日やけ止めの代わりではないということです。
日やけ止めは、これからも欠かせません。

Beyond Sunscreen—Protecting the Skin Within

This week I read an article in The Washington Post that stopped me in my tracks.

Basal cell carcinoma is the most common type of cancer, affecting an estimated 1 in 5 adults in the United States during their lifetime. Although it is more common after the age of 40, younger people are certainly not immune.

The article told the story of a 23-year-old woman who noticed a small pink, flaky spot on her nose. She thought it was an acne scar or perhaps eczema. It turned out to be basal cell carcinoma.

That was what caught my attention. Basal cell carcinoma does not necessarily look frightening. It can begin as something as innocent-looking as a small bump, a sore, or a scaly patch that could easily be mistaken for dry skin or a pimple that simply isn't healing. It is rarely life-threatening, but it can grow deeper into surrounding tissue, which is why early detection is so important.

This subject is also personal for me.

My brother has had basal cell carcinoma. He is an avid golfer—out on the golf course practically seven days a week—and today he is much more conscious about applying sunscreen when he is outdoors.

Recently, I also gave him my ninoBeauty Protexidine Defense and Protexidine Eye Cream, which he now uses every day.

What makes me happy is that he isn't using them simply because his sister gave them to him! He genuinely likes what they are doing for his skin. He tells me that since using the Eye Cream, the bags under his eyes seem to be improving. Recently, a Japanese dermatologist told me that she believed skin cancer would become an increasingly serious concern in Japan.

And yet, perhaps nowhere do you see people as sun-conscious as you do here. On a clear summer day in Japan, there are umbrellas everywhere!

But an umbrella alone is not enough.

Dermatologists recommend broad-spectrum, water-resistant sunscreen of SPF 30 or higher, and when we are outdoors, sunscreen needs to be reapplied approximately every two hours—and after swimming or sweating.

This is where I think the conversation about sun protection becomes particularly interesting.

SPF is our first and essential defense. But what happens to the small amount of UV radiation that still reaches the skin?

That question is one of the reasons I became so interested in ProteXidine.

Research on ProteXidine has looked at its ability to help reduce several damaging effects caused by UV exposure, including damage involving cyclobutane pyrimidine dimers—or CPDs—which are lesions created in DNA after UV exposure. Importantly, the published research to date includes an ex vivo skin model, so I believe we should talk about this promising science accurately and responsibly.

Steve Hernandez, Head of Research and Development at Topix Pharmaceuticals, once explained the concept to me as three lines of defense:

  • First line of defense: Filter as much UV light as possible with sunscreen.
  • Second line of defense: Use antioxidants to help address the oxidative stress created by the UV that gets through.
  • Third line of defense: Support the skin at a deeper cellular level against UV-related damage.

I love this explanation because it makes something very complicated quite simple.

And it is important to understand that ProteXidine does not replace sunscreen. Sunscreen remains essential. ProteXidine Defense is another part of the skincare routine—one designed to complement good daily sun protection, not substitute for it.

For me, protecting our skin is no longer just about avoiding a sunburn or preventing wrinkles.

It is about understanding that what we do to our skin every single day matters.

Wear your sunscreen. Reapply it. Wear your hat—or your Japanese sun umbrella! Pay attention to any spot on your skin that changes or simply doesn't heal. And if something concerns you, see a dermatologist.
